Questionnaires

To modify your questionnaires, click on the Questionnaires icon in the top right corner of the Store Audits page.

The system will present the list of questionnaires which have been created in your franchise.

Click on a questionnaire to open the questionnaire editor. The top panel will list some basic information about your questionnaire. The goal of a field audit is to assign a % to a visit. The passing score % is the minimum score required to receive a passing score.

If your questionnaire has already been used in the wild, the next panel will feature analytics. The questionnaire is presented with the average scores. You can use the convenient filters at the top to filter the data in many ways. One of them is a date range filter, allowing you to focus only on the most recent results.

The second tab in the analytics panel shows you the actual results of each audit. Clicking on one of the rows will open the full audit in a pop-up window.

To view the questionnaire editor itself, as if you had no results on this audit questionnaire, click on the Edit Anyways button.

The questionnaire editor itself is fairly straightforward. It lists sections which include questions of various types. The editing tab lets you add/modify/remove questions & sections. The ordering tab lets you re-order elements easily whereas the preview tab gives you a simpler interface to review the questionnaire template without seeing all the configuration settings.

Questions can be of many types as seen below.

  • Yes/no questions are self-explanatory. They are the most common questions in a field audit.
  • Text presents a text box with a single line, allowing for short answers. Text area questions are very similar, but the auditor can include multiple lines in their answer. Text fields are used for annotations; they do not impact the final score.
  • Multiple choice questions can be configured to only allow a single option to be selected. (Ex: How many cups do you have? A) <100, B) 100 to 500 c) 500+) Multiple selections are also supported (Ex: Select up to date marketing elements A) sign B) menu C) window decal). Each answer can be given a different weight.
  • Images can also be uploaded to complement answers. Images to not impact the audit's final score.

The advanced mode (which can be enabled by clicking the Advanced mode at the top or the ... button which appears when you hover over a question) lets you configure advanced questionnaire elements. These are hidden by default as they are not typically necessary.

  • Points indicate the weight of the question.
  • Yes is typically the correct answer in an audit, but you can choose otherwise with the 'No' is the right answer checkbox.
  • Auditors can create tasks when filling out an audit. The 'Associate task to' dropdown lets you choose the default recipient of a task created for this question. (Example: If a question asks if the marketing material is up to date, follow-up tasks could default to the marketing department if they are the ones responsible for sending the material)
  • Documentation is a bit of background information on the question. This information will be shown while viewing/answering an audit. Documentation can include hyperlinks to your Operations manual or even pictures of expected results.

Questionnaires can also be translated by changing the language in the dropdown menu at the top of the editor. Fields which need to be translated will have a blue border as shown below.
