Create

2 x Name fields
2 x company fields.

Concatenate:

Phone 1 with Phone 1 label
Phone 2 with Phone 2 label
Email 1 with Email 1 label
Email2 with Email 2 label
Street, City, State, Postal Code and Country

Find and Replace
Replace the comma (,) in the keywords column with a semi colon (;)

Save the file as an Excel workbook.
